INSTRUCTIONS FOR Patients

Overview

Were you forced to move from your home by the recent floods?  Have you run out of medications or cannot remember which prescription drugs you need to take or how many pills a day?  Have you been unable to contact your regular doctor, clinic, or hospital?  If so, calling 1-877-788-5888 may be helpful to you.

The Indiana Family & Social Services Agency, Regenstrief Institute, EDS and the Indiana Health Information Exchange are providing the Indiana Flood Victim eHealth Support Center (1-877-788-5888) that allows licensed doctors who are currently treating a flood victim to receive information about that patient's medical history.  These calls are answered by staff at the Indiana Health Information Exchange, a non-profit that works with the medical community throughout Indiana . 

This secure service could help your doctors know which drugs you have been prescribed, what recent tests you've had completed, along with other limited medical information.  It complies with all state and federal privacy laws.  If the flooding has limited your providers access to your medical records and you are under a doctor's care, just tell your doctor to call the Indiana Flood Victim eHealth Support Center at 1-877-788-5888.  By letting your doctors know about this service, they can do more to help you.
