INSTRUCTIONS FOR DOCTORS Overview With the assistance of the Indiana Family & Social Services Agency, Regenstrief Institute, EDS and the Indiana Health Information Exchange, the Indiana Flood Victim eHealth Support Center (1-877-788-5888) may be able provide you with a summary of your patient's recent medical information through the Indiana Network for Patient Care. This secure clinical data repository complies with all state and federal privacy laws. By calling 1-877-788-5888, the Indiana Flood Victim eHealth Support Center team can query the Indiana Network for Patient Care which provides a summary of the information may be available about your patient to the extent obtainable. Not all patients will have information in this system, however. The information provided is meant to complement, not substitute for, complete medical information provided to the physician by his or her patient. Medical information pertaining to selected sensitive health care conditions is not available through this summary.
The information will be sent to the physician office once the practice information has been verified. Medical information, to the extent obtainable, will be faxed to your office as soon as possible after the initial request. The toll-free number (1-877-788-5888) is staffed and available 24/7 to accept requests for medical information. Information requests during business hours (7 a.m. – 5 p.m. Eastern) will be fulfilled in less than one hour. Calls received after business hours will be fulfilled the next business day.
