INSTRUCTIONS FOR DOCTORS

Overview

Many Indiana patients' medical records have been made unaccessible by the recent flooding.  There is a critical need to provide physicians who are treating them with patients' medical history from alternative sources to avoid delays in treatment and potential complications of health due to the inability to obtain medical information. 

With the assistance of the Indiana Family & Social Services Agency, Regenstrief Institute, EDS and the Indiana Health Information Exchange, the Indiana Flood Victim eHealth Support Center (1-877-788-5888) may be able provide you with a summary of your patient's recent medical information through the Indiana Network for Patient Care.  This secure clinical data repository complies with all state and federal privacy laws.

By calling 1-877-788-5888, the Indiana Flood Victim eHealth Support Center team can query the Indiana Network for Patient Care which provides a summary of the information may be available about your patient to the extent obtainable.  Not all patients will have information in this system, however. 

The information provided is meant to complement, not substitute for, complete medical information provided to the physician by his or her patient.  Medical information pertaining to selected sensitive health care conditions is not available through this summary.

In order to request information about a patient you are treating, you will need to provide as much of the patient information below as possible:

• first name
• last name
• date of birth
• pre-flooding residence address
• Social Security Number
• Insurance Carrier

The information will be sent to the physician office once the practice information has been verified.

Medical information, to the extent obtainable, will be faxed to your office as soon as possible after the initial request.  The toll-free number (1-877-788-5888) is staffed and available 24/7 to accept requests for medical information.  Information requests during business hours (7 a.m. – 5 p.m. Eastern) will be fulfilled in less than one hour.  Calls received after business hours will be fulfilled the next business day.

Information that may be obtainable includes:

• Lab, radiology, pathology reports and other results
• Medical information derived from:
• Commercial insurance claims
• Medicaid claims
